Terms and Conditions
1. Cancellation Policy
- A 90‑day commitment applies after starting any new therapy, including new medications added later in treatment. This is to preserve the quality and integrity of our treatment plans and is in your best interest.
- After 90 days, you may cancel anytime with written notice before your next billing cycle.

2. Billing and Payments
- Membership fees are billed monthly to the payment method on file.
- Payments are processed automatically each month, on the sign-up anniversary date.
- You are responsible for keeping your payment information up to date.
- Failed payments may pause services until resolved.

3. Clinical Care and Eligibility
- Your membership includes access to clinical evaluation, treatment planning, and ongoing monitoring.
- All care is based on clinician judgment and medical appropriateness.
- Some therapies require labs or follow‑up visits. Labs are not included.
- Clients may use insurance for labs or choose a cash option.
- Ember may discontinue treatment if it becomes unsafe or clinically inappropriate.

4. Communication and Access
Members receive access to:
- Four telehealth visits in the first year.
- Two telehealth visits per year starting in year two and beyond.
- Messaging through the patient portal.
- Ongoing support and monitoring.
- Ember also provides a state‑of‑the‑art symptom tracker delivered at regular intervals. This allows real‑time symptom management and timely treatment or medication adjustments by our clinical team. This benefit is unique to Ember.
Response times may vary based on clinical volume, but we aim to respond promptly and thoughtfully.
5. Medication Policies
- All medications prescribed through Ember follow state and federal regulations.
- Prescriptions require appropriate evaluation and monitoring.
- Medication costs are not included in membership fees.
- Medications can be sent to the pharmacy of your choice.
- Some medications may be eligible for insurance coverage depending on your plan.

6. Refunds
- Membership fees are nonrefundable.
- No refunds are issued for unused services, partial months, or retroactive cancellations.
- Billing errors will be reviewed promptly.
7. Privacy and Confidentiality
- Your information is protected under HIPAA.
- We do not share your information without consent unless required by law.
- A copy of our Privacy Policies is posted here and a copy is available to you upon request.

8. Changes to Membership or Pricing
- Ember may update membership pricing or program features in the future.
- You will be notified in advance of any changes.
9. Community Standards
Ember is a community built on respect and safety. Membership may be discontinued for:
- Abusive or inappropriate behavior toward staff or clients.
- Misuse of services.
- Noncompliance that compromises safety.
